Advance Care Planning Open Houses In-Person
The Northfield-based End-of-Life Choices Collective will offer information about advance care planning and individual assistance with health care directives. Trained advance care planning facilitators will be on hand to help you navigate this process. Free and open to adults of any age. Materials provided, or you may bring your own form. Open house format from 4:30-6:00pm.
